Your consent is required to process certain Personal Data (as defined in Sword’s Privacy Statement) that is required to provide the Sword Health health and wellness services (“Services”). Sword may collect and use the following data, in accordance with our Privacy Statement in order to provide the Services: information and medical history you provided to us, your medical records, images, information gathered or created during your telehealth visits, demographic data, your physical address, email address, IP address, telephone number, and output data collected by the devices you use as part of the Services. 

As part of the Services, certain devices will be connected to your personal Sword account. The data collected from these devices will include movement and motion tracking information and your voice. Your consent is required for this pairing. 

You have the right to withdraw your consent at any time. If you withdraw your consent, you will not be able to receive the Services.
