Bunion

Carolina Moreira

A bunion (hallux valgus is the medical term) is a bony bump that forms at the base of the big toe when the joint shifts out of alignment, causing the toe to angle toward the others.

What a bunion does to your foot and how it affects the way you move

A bunion develops at the metatarsophalangeal joint — the joint at the base of your big toe — when the bones gradually shift so the big toe points inward and the joint protrudes outward. That bump isn't a separate growth; it's the displaced joint itself. Over time, the skin over it can become red, swollen, and tender, and the joint may become stiff or painful with walking.

Because the big toe plays a central role in pushing off with each step, a bunion can alter your gait in ways that create secondary problems in your foot, ankle, knee, or hip. Ill-fitting footwear tends to make things worse, but footwear alone doesn't cause a bunion — the structural shift is usually driven by a combination of foot mechanics, genetics, and load over time.

Why bunion pain is often undertreated

Many people assume that a bunion can only be fixed with surgery and avoid seeking care in the meantime. Conservative management — including targeted exercises, footwear adjustments, and addressing the movement patterns that increase joint load — can meaningfully reduce pain and slow progression, even when the structural change doesn't reverse. Starting that process earlier rather than later makes a real difference.

Why the pain doesn't always match the size of the bump

A bunion visible enough to notice on an X-ray isn't always painful, while a less prominent one can cause significant discomfort. How much the joint is irritated, how the surrounding muscles are compensating, and whether the altered gait is loading other structures are often bigger drivers of pain than the size of the bump itself — and those factors respond well to physical therapy.

How Sword Health can help

A physical therapist can evaluate how your foot is moving, address the muscle imbalances contributing to joint stress, and help you manage bunion pain without waiting for symptoms to become severe. Sword connects you with that care from home, on a schedule that fits your life.
