The quadriceps are a group of four muscles at the front of the thigh that work together to extend the knee and help control the leg during walking, running, squatting, and landing.

What your quadriceps do and what happens when they're weak

Your four quadriceps muscles — the rectus femoris, vastus lateralis, vastus medialis, and vastus intermedius — converge into the quadriceps tendon, which connects to the patella and then continues as the patellar tendon down to the tibia. Their primary job is straightening the knee, but they also control the rate at which the knee bends during landing and descending stairs — an eccentric function that's critical for protecting the joint from impact.

Weakness in the quadriceps, particularly in the vastus medialis on the inner side, alters how the patella tracks and increases the compressive forces on the joint with every step. After knee surgery or injury, quadriceps atrophy occurs rapidly and is one of the primary targets of rehabilitation — because without adequate quad strength, the knee can't be loaded safely and the recovery timeline extends significantly.

Why quad strength is the foundation of knee rehabilitation

Almost every knee condition — osteoarthritis, patellofemoral pain syndrome, ACL reconstruction recovery, meniscus repair — involves quadriceps strengthening as a core element of the rehabilitation plan. The quadriceps aren't just about power; they're about control, and restoring that control is what allows the knee to function reliably under load.

Why some quad exercises do more harm than good in the early stages

Open-chain quad exercises like straight-leg raises place isolated load on the quadriceps without compressing the knee joint. Closed-chain exercises like squats and step-downs load the quads in more functional ways but create joint compression — which isn't appropriate for every knee condition at every stage.
